2日、米FDAの非処方せん薬諮問委員会（Nonprescription Drugs Advisory Committee）が開催され、モンテルカスト（本邦販売名：シングレア、キプレス）のOTC品を承認するかどうかの審議が行われました。

承認の適応症は、18歳以上の hay fever （花粉症） または上気道のアレルギーで、ラベル（→該当ページにリンク）には”DO NOT USE TREAT ASTHAMA” とはっきりと記されるそうです。
今回も大手メーカーの特許切れの大型商品のスイッチの試みですが、諮問委では有効性や精神神経系のイベントなどの安全性や、未成年や適応外（喘息）に使用される潜在的なリスクはないか等が話し合われました。（→ドラフト議案）
また、午後の公聴会では、医薬品監視団体のPublic Citizenがスイッチに反対する証言を行っています。（日本ももっとオープンな会議にできないのだろうか？）

Public Citizen では、
- FDAが承認した既存のOTCアレルギー薬と比べて有用であるというエビデンスはない
- 他の治療法と組み合わせた場合も、付加的な利益を提供するエビデンスはない
- 興奮、攻撃行動、不安、抑うつ、幻覚、不眠、神経過敏や自殺願望といった副作用など、既存のOTCアレルギー薬と比較して明らかに高いリスクがある。
として、スイッチは拒絶されるべきとしています。

審議を受けての諮問委では、11対4で承認を支持しないとする評決結果となり、FDAが現時点の申請内容で承認を行うことは難しくなりました。（まれに、諮問委の意見を覆して承認することもあるけど）
やはり、精神神経系の副作用が軽視できないことや、喘息への自己判断での使用の懸念が拭えなかったためのようです。（他にはグレープフルーツジュースとの相互作用の懸念や、頭痛（本邦でも記載はある）や夜尿症といった副作用もあるらしい）
